Nurse Cherish provides private-pay concierge nursing services intended to support, educate, advocate, and coordinate care for clients. Services are provided by a licensed registered nurse and are non-emergency, non-insurance based, and supplemental in nature. Concierge nursing services do not replace primary care, specialty medical care, home health services, emergency services, or inpatient care. Clients are encouraged to maintain an established relationship with their primary care provider and appropriate medical specialists at all times. Nurse Cherish does not provide medical diagnosis, prescribe medications, or alter treatment plans as ordered by a license physician or advanced practice provider. Any health education, recommendations, or observations provided are for informational and supportive purposes only and are not intended to constitute medical advice. Concierge nursing services are not designed for urgent or life-threatening conditions. In the even of a medical emergency, clients are instructed to call 911 or immediate emergency medical care. Services are offered on a private-pay basis only and are not billed to insurance, medicare, or medicaid. Clients are financially responsible for all services rendered. Concierge nursing availability is not continuous or on-call unless explicitly stated in a written service agreement. Response times are not guaranteed unless otherwise specified in the client's membership plan.All services are provided within the scope of practice of a registered nurse under Illinois and Wisconsin law. No guarantees are made regarding health outcomes. By engaging in concierge nursing services with Nurse Cherish, the client acknowledges understanding and acceptance of the above terms and limitations.
